ICO: Security breach notification form available online

The ICO has made available, on its website, an online form for informing the office about data breaches. The ICO wants to know about any previous incidents, whether any personal data is at risk, how many people have been affected, whether your organisation notified the affected individuals of the incident, etc. 

While the ICO is encouraging organisations to notify electronically, it says it cannot guarantee the security of the forms sent by email. Having received a breach notification, the ICO will contact the organisation within seven calendar days.
